Intracytoplasmic Sperm Injection (ICSI) is a treatment for infertility mostly used as a part of in vitro fertilization (IVF) to help couples conceive. In ICSI, single sperm is directly injected into each of the retrieved eggs through micromanipulation equipment rather than general infusion as part of normal IVF. Once the embryo/s are formed, it is placed directly inside the mother’s uterus.

IVF and ICSI are very similar in the sense that in both treatments eggs and sperm are retrieved for external fertilization using artificially clinical techniques. However, in ICSI, specific sperm cells are injected into the egg as against general IVF where sperm and eggs are mixed freely together.

  1. When male partner has severe infertility issues such as low sperm concentration (less than 15-20 million/ml) or is facing an issue of low sperm motility (less than 35%).
  2. ICSI can be done when the male is inflicted with Azoospermia (zero sperm in ejaculation) or Teratospermia (sperm with abnormal morphology).
  3. Men with a history of vasectomy.
  4. Men with issues of retrograde ejaculation (ejaculation of sperm in urinary bladder). In such cases added sperm retrieval techniques such as TESA (Testicular sperm Aspiration), MESA (percutaneous epididymal sperm aspiration) or PESA (percutaneous epididymal sperm aspiration) is used to retrieve sperm for insemination.
  5. After an IVF history, with the female partner having no or low percentage of mature eggs to get fertilized.
  6. Low yield of eggs during egg retrieval stage as part of IVF. In such scenarios, ICSI is suggested, as low yield eggs are not compatible for natural fertilization.
  1. Ovarian stimulation -

    as a first step, the ovaries of the mother are stimulated with medicines to help the growth of follicles in the eggs. Post this the response of the ovaries and final maturation of the eggs is closely monitored.

  2. Retrieval of eggs -

    the mature eggs are retrieved within 35-38 hours under sedation.

  3. Sperm selection -

    motile and healthy sperm are selected for insemination.

  4. Assessment of eggs-

    the eggs are surrounded by cumulus cells which are removed through delicate enzymes so that they can be finally prepared for insemination.

  5. Insemination-

    Most healthy sperm is injected into the matured egg.

  6. Verification-

    the eggs are tested next morning to see if they got fertilized or not.

  7. Transfer of the embryo-

    the embryo, which is chosen for transfer, is passed through the cervix into the uterus through transfer catheter. Normally one embryo is transplanted however in some cases transfer of more than one embryo happens.

  8. Pregnancy test-

    After 2 weeks of embryo transfer, pregnancy blood test is done to see if the ICSI procedure has been successful or not.

  1. ICSI is beneficial for those couples who have failed multiple IVFs before. Approximately 50-80% of the eggs get fertilized during ICSI.
  2. It benefits the couple that desires pregnancy post-vasectomy.
  3. ICSI is for those males who have lower sperm count and mobility.
  4. Beneficial for males with retrograde sperm ejaculation (sperm ejaculated in the urinary bladder).
  1. Damage to eggs can happen during retrieval.
  2. Multiple gestations can be a problem after ICSI. Also preterm labor and baby with low weight cases can happen.
  3. Gestational diabetes, low amniotic fluid levels and high blood pressure in the mother can also be seen at times.
  4. Only 0.4 % babies through ICSI have been reporter to have birth defects.
