        Dr Mohit is the best fertility expert in Mumbai. I had very low sperm count and was feeling very low because I was not able to make my wife pregnant. But doctor Mohit gave me enough confidence and started our IVF treatment. My wife is 6 months pregnant now and we will be welcoming our little baby very soon.

      Q1. I am recently diagnosed with PCOS. Can it affect my fertility and my chances of conception?

      • A. Many women with PCOS (Polycystic Ovary Syndrome) struggle to conceive due to anovulation (no ovulation) or very irregular and infrequent ovulation. Although complete cure is not possible in PCOS; most women with PCOS can have healthy pregnancies with many treatment options (medication, lifestyle changes) depending on the specific requirements. In extreme cases where natural ovulation does not occur even after treatment, a woman with PCOS can choose to have a child through ART (Artificial Reproductive Technology) procedures such as IVF (In-vitro Fertilisation).

      Q2. Are there any restrictions on physical or personal activities during an IVF cycle?

      • A. Yes, there are some restriction to follow before and after IVF treatment to improve the chances of implantation and to carry a pregnancy without any problems. You should avoid heavy exercises such as aerobics, jogging or weight lifting during ovarian stimulation and until the pregnancy test results are known. Smoking should be stopped before ovulation induction begins and it is highly recommended to discontinue tobacco intake at least 3 months before an IVF cycle. Drinking alcohol is not advisable from the time fertility medications are initiated until the pregnancy test is conducted as it may cause side effects. As some medications may interfere with the fertility medications prescribed, some are not safe to use before an operation or medical procedure. Common medications can also interfere with ovulation or pregnancy implantation. All prescription or over-the-counter medications you are taking must be informed to the doctor. Herbal supplements should also be during IVF cycles. Do not skip doses and do not decide to stop your medication prescribed by your infertility specialist due to minor side effects. Keeping your body temperature at a normal level is also advisable as too much heat is not ideal for embryo implantation. Therefore, it’s best to avoid hot baths as well as keeping out of hot tubs and saunas for the duration. They can also make you vulnerable to infections.

      Q3. Is painless delivery safe for me and my baby?

      • A. During painless delivery, an epidural injection is used to decreases pain in the pelvic region. Epidural anesthesia is the most popular method of pain relief during labor. Women often choose to have one as it reduces pain during delivery of a baby. It is usually given when there are complications during childbirth, such as those resulting in cesarean delivery. An epidural is delivered through a tube via the spine which takes about 10 minutes to administer and an additional 10 to 15 minutes to work. The benefit of an epidural is that it allows painless delivery. Even if you are still aware of the birth and you may still feel contractions, the pain is decreased significantly. An epidural is also required in a cesarean delivery to ease pain from surgically removing a baby from the womb. In some cases, general anesthesia is used as well, where the mother is not awake during the procedure. While some cesarean deliveries are elective, most are required if vaginal delivery cannot be accomplished. Even if they are rare, an epidural procedure has some risk factors which include back pain and soreness, headaches, persistent bleeding, fever, breathing difficulties, drop in blood pressure, which can slow down the heart rate of the baby. The fact that mothers cannot feel all of the elements of delivery with an epidural can also lead to a host of other problems, such as the increased risk of tearing during vaginal delivery. Risks with cesarean deliveries are not necessarily related to the epidural.

      • A. If you are considering undergoing IVF treatment for infertility, it is natural for you to have a few questions. Is IVF painful? is one of the most common questions asked by most patients before they undergo the treatment. Although, the answer to that the discomfort and will provide comfort through the procedure. In most cases, IVF injections are not much painful. There is a stinging sensation but that is not anything to be bothered about. The needles are too thin to cause any pain. You can ask your partner or even your friend to stay by your side when you administer the drugs through injections. However, there might be some discomfort after egg retrieval. After the removal of eggs, it is common to experience some cramping. However, this cramping is similar to the menstrual cramps that women experience every month since their puberty. The transfer of embryo is done with the help of a small catheter. While it might sound scary and intimidating, there is nothing for you to worry about. If you have come this far with the treatment, you will be able to go through this step as well.

      Q5. What tests can we go for to find the possible causes of unexplained infertility?

      • A. A couple is said to have unexplained infertility only after a complete fertility evaluation of both the male and female partner. While a doctor may diagnose your case as unexplained, a fertility specialist may say you just have not been fully evaluated. Before you approach, you and your specialist need to have a good understanding of the possible root causes of your infertility. In many cases, there is more than one factor for infertility. Diagnostic tests are an important tool which can reveal both female and male factor infertility. About 10% of couples are diagnosed with unexplained infertility because no specific cause can be identified. One in five cases of infertility is unexplained even after a full diagnostic workup. Even if the reason for infertility cannot be identified, the test results are still valuable information which your doctor can use to help you create a treatment. It can be caused by undiagnosed underlying medical problems, mild endometriosis, the interaction between the vaginal environment and sperm, poor egg or sperm quality, problems with the endometrium or unhealthy embryo. Other underlying conditions that may cause infertility include an undiagnosed thyroid disorder, diabetes, or some autoimmune illnesses. While some doctors provide some form of treatment to help assist pregnancy, some couples with unexplained infertility will conceive without any treatment help within one to two years of diagnosis. If your doctor diagnoses you with unexplained infertility and tells you to need more time, you may also want to consider a second opinion, such as using donor eggs or sperms. IVF has a high rate of success for young women with unexplained infertility as they have normal ovarian reserve results, therefore, it is worth trying. For older women and men, the options can be using donor embryos or surrogacy.
