Intrauterine Insemination is an infertility treatment, which involves artificially inducing sperm inside the uterus of a woman’s womb to lead to fertilization and pregnancy. IUI is done when not enough partners’ sperm cells are able to reach the fallopian tube naturally.
  1. If the mother fails to ovulate regularly due to PCOS/PCOD (Polycystic Ovary Syndrome/Disease).
  2. The Male partner is detected with low sperm count (less than 60 million/ml) as part of Semen Analysis test.
  3. Mother having problems with cervix or cervical mucus in form or texture and in turn impacting the right environment for the sperms to travel.
  4. Couples with sexual dysfunctions (problem to provide sexual response i.e. excitement, orgasm) or infrequent coitus (sexual intercourse) rate.
  5. When the couple has unexplained fertility problems (no detectable cause for infertility) and are not being able to conceive for more than 6 months of trying.
  1. The average success rate of IUI (across all types) is as high as 18% per cycle.
  2. IUIs with fertility drugs used is much more successful than natural IUI (no drug used IUI). Natural IUI has average success rates of merely 6 %.
  3. IUI is dependent on the sperm count and motility. If the male counterpart has motile sperm count of less than 5 million/ml at the time of processing, chance of IUI success is very low.
  4. Women with PCOS/PCOD (Polycystic Ovary Syndrome/Disease) have relatively low chances of IUI success.
  5. Women with problems in both fallopian tube blockage (Hydrosalpinx, Pyosalpinx, Hematosalpinx, Chronic Salpingtis etc.) have normally very less chance of conceiving through IUI and they should normally go for IVF.
  6. Women after 40 have low chances of IUI success due to depleting ovarian egg reserve and egg quality.

IUI procedure involves three variations depending on the ovarian health of the woman:

  1. Natural cycle IUI - Natural ovulation is detected using ovulation prediction kits. After she ovulates, insemination is done within 6 hours.
  2. Monitored cycle IUI - Under this procedure, ovaries is observed through ultrasound over a period of few days before expected ovulation. Once at least one mature egg develops, medicinec is injected for the complete ovulation of the egg and IUI insemination is planned for the next day.
  3. Medicine-assisted cycle IUI - Medicine is given if the woman doesn’t ovulate regularly. Once she ovulates, IUI is planned. Fertility drug induced IUI is performed normally after 24-48 hours of ovulation.
  1. IUI is a non-surgical and simple painless procedure under ART (Assisted Reproduction Technology)
  2. Less expensive than IVF or any other ART (Assisted Reproductive Technology) treatments. One cycle of IUI costs around $900 (in US) as compared to IVF of $16,000 per cycle. In India the average cost per IUI cycle is roughly around to 10000 INR.
  3. Less stress on the body as gentle medicines and simpler treatment is used. With IUI you also have the option to go for a natural cycle procedure, to protect your body more from hormonal imbalances.
  4. With IUI, chances of getting pregnant are twice as high as natural sex. This is because the procedure involves selecting the most motile and healthy sperm for insemination.
  5. Less time consuming as it only takes 45-60 minutes for the procedure.

Apart from care and consultation, Gynecologists perform various surgeries as well depending on the nature of the gynecological problem. Common surgeries/operations performed by a Gynaecologist are-

  1. Usual success rates of IUI are around 18% per cycle depending on the age, fertility reason and fertility drug usage. This is low compared to IVF from reputed IVF specialists.
  2. Risks of getting pregnant with multiple babies.
  3. Premature delivery and low baby weight can occur.
  4. Cramping, spotting; and in some cases severe bleeding can also be seen.
  5. If you have a history with Pelvic Infection Diseases -PID (Sexually transmitted PID, Subacute PID, Chronic PID), IUI can be dangerous.
  6. The fertility drugs prescribed for IUI can cause Ovarian Hyper Stimulation Syndrome with signs such as nausea, bloating, diarrhea and weight gain.
  7. Some of the common side effects of IUI can be observed as hot flashes, mood swings, anxiety, vision problems, breast tenderness and in extreme cases growth of ovarian cysts.
  1. Stimulating the ovaries- The IUI doctor checks up the woman’s hormonal conditions including ovulation along with examining the supportive ability of the uterus to hold a pregnancy. Depending on the ovarian health of the woman, fertility drugs are given to her for healthy egg production.
  2. Sperm retrieval- The Male partner’s sperm sample is taken and analysed.
  3. Sperm wash- Retrieved healthy sperm (in cases of Donor the Donor Sperm) is washed to remove all the toxic elements, which can risk the pregnancy. The washing procedure helps in selecting the healthiest and most mobile sperms for the process.
  4. Insemination- During the day of IUI procedure the IUI specialist injects a very small and flexible catheter in the cervix through which the Sperm is transmitted to the women’s ovaries.
  5. Pregnancy Scan- The next follow up falls after a week and progesterone levels in the mother is checked. The IUI specialist examines the thickness in the uterus lining and accordingly makes sure if the woman is pregnant or not.
