    IUI or Intrauterine Insemination is a type of artificial reproductive treatment, in which sperms are injected into a woman’s uterus or fallopian tubes when she is ovulating. IUI treatment is less invasive and cheaper than IVF. IUI is generally performed using the intended father’s sperm or donor sperm. To encourage fertilization, the treated sperms are placed at close proximity to the egg inside the uterus of the woman so that they can fertilize the egg. (1)

    Reasons for IUI Treatment

    IUI treatment is recommended when any of the following condition arises:

    1. Male Infertility

    Male infertility usually occurs, due to either low sperm count or decreased motility. In some men, sperms can be too weak to travel to the fallopian tube to fertilize the egg. Male infertility can be a result of an unhealthy lifestyle, smoking, alcoholism, exposure to harmful substances, illnesses, chronic health problems or injuries. Around 90 percent of men suffer from infertility issues due to low sperm count. (2)

    2. Hostile Cervical Mucus

    Hostile cervical mucus or infertile cervical mucus is one of the reasons for a woman not being able to conceive. It is needed for sperms to survive and swim through cervical mucus while traveling from the cervix to fallopian tubes. Hostile cervical mucus occurs when there are problems with vaginal secretions or vaginal dryness. This can be due to the use of birth control methods or hormonal imbalances. (3)

    3. Unexplained Infertility

    If the cause of infertility is unknown, it is known as unexplained infertility. Unexplained infertility can be associated with both male and female infertility issues. (4)

    4. When fertility Drugs are not Successful

    Fertility drugs may help a woman conceive but they are not effective for everyone. IUI treatment is performed when other fertility treatments and drugs fail to provide the desired results. (5)

    5. In Case of Donor Sperms

    If the male partner is not able to produce sperms or do not have the quality of sperms to conceive, the sperms from a donor are used to fertilize the female egg. (6)

    6. If Sexual Pain makes Intercourse Impossible

    Some couples may face problems during sex. Problems may be because of extreme vaginal pain or male ejaculatory dysfunction during sexual intercourse. This can lower the chances of conceiving through natural methods, so the only way of fertilizing the egg could be through Intrauterine Insemination (IUI). 

    7. When to Avoid IUI

    IUI is not recommended for those with:

    • Blocked fallopian tubes
    • Severe endometriosis
    • Previous pelvic infection

    Some hospital and specialist may recommend a few cycles of IUI before paying for IVF treatment. 

    Intrauterine Insemination (IUI) Procedure

    When a patient undergoes the IUI procedure, they are expected to go through the following three steps:

    • Firstly, the patient is given fertility drugs and ovulation cycle is monitored.
    • The male partner provides a sperm sample (or it can be obtained from a donor) that is specially washed and prepared for the insemination.
    • Finally, the doctor injects the prepared sperm into the uterus. (7)

    IUI Procedure for Male Infertility

    During this evaluation, a doctor checks the male partner’s semen for sperm count and motility. The male partner is asked to give several semen samples in case the sperm count is low. These semen samples are further prepared to improve the sperm count. They are washed and treated to remove any kind of toxic chemicals. The motile and healthy sperms are selectively prepared to increase the success rate of the IUI procedure. The doctor may also advise avoiding ejaculation for two to three days to increase sperm count. (8)

    IUI Procedure for Female Infertility

    Before the procedure, the woman needs to visit a fertility specialist and do certain tests. These tests determine the patient’s health, hormone levels and the ability of the patient to support a pregnancy. Next, the doctor gives ovulation drugs to produce healthy eggs while regularly monitoring through ultrasound. (9)

    Inserting Sperms during IUI

    The insertion of sperms during the IUI treatment requires a maximum of one to two hours. Sperms are inserted when the luteinizing hormone (LH) surges before ovulation. The female partner lies down on the examination table as a very thin and flexible catheter is inserted into her cervix. The treated sperm sample is directly injected inside the uterus or fallopian tube through the catheter.

    After a week, the doctor checks the progesterone levels. After the IUI process, the doctor prescribes a progesterone hormone to assist pregnancy. With an ultrasound, the doctor observes the thickness of the uterine lining. By checking the thickness of the uterus lining, the doctor may have some idea as to whether the IUI procedure was successful. The doctor may also do blood tests to examine the level of progesterone and estrogen. After 14 days, he may again call the patient for other pregnancy tests.

    Success Rate of IUI

    The success rate of IUI depends on the cause of infertility and age. The success rate of IUI treatment can go up to 80 percent with three to six cycles of IUI procedure. When IUI treatment fails after several attempts, the doctor may recommend IVF treatment (In Vitro Fertilization) or surrogacy, which is much effective than IUI. Though the success rate of IUI is not as high as IVF treatment, IUI is easier to perform and less expensive. (10)

    IUI Treatment Cost

    Infertility treatments in India are also performed through medications and fertility drug use but the success rate for medical treatment is low. IUI treatment cost is higher than treatment through medication. Although IUI offers a higher success rate, it is less effective as compared to IVF. IVF treatments are expensive but provide a higher success rate of pregnancy. The cost of treatment may also depend on the specialization of the doctor, the reputation of the hospital and the region where the patient is treated. (11)

    IUI treatment involves a lesser risky procedure but still has some small risk of infection. Side effects of IUI caused by fertility drugs are mild and often disappear once the medication is stopped. The risk of becoming pregnant with twins, triplets, or more children with IUI is small, and this is a side effect of fertility drugs taken with IUI. The risk of developing Ovarian Hyperstimulation Syndrome (OHSS) and becoming pregnant with multiples is ten percent for women who take ovulation-inducing drugs and 30 percent for women who take gonadotropins. (12)
