IUI treatment step by step is executed for infertile couples for male infertility treatment, Endometriosis, and Female infertility treatment. IUI drugs include hCG injections, Gonadotropin, and Clomid. The 3 levels of monitoring for IUI procedure includes first, second, and final level of monitoring.

Let’s Discuss in More Details About Drugs, Monitoring, and Procedure for IUI:

Intrauterine insemination (IUI) is a fertility treatment in which sperms are prepared and delivered directly into a woman’s uterus when she is ovulating to facilitate fertilization. This procedure helps sperms get closer to egg, increasing the number of sperms that reach fallopian tubes thereby increasing the chances of fertilization. Unlike IVF, during IUI, a sperm still has to reach and fertilize egg on its own. IUI is a less invasive and inexpensive method compared to In vitro Fertilization.

Candidates for IUI

Intrauterine insemination treatment is used by couples who have:

1. Endometriosis:

Patients suffering from endometriosis related infertility are administered medications to obtain a good quality egg. When eggs are mature to be fertilized, IUI is performed.

2. Donor Sperm:

For couples with male infertility treatments, donor sperms are used. These donor sperms are commonly used to achieve pregnancy through IUI treatment. Frozen semen samples are obtained from freezing centres and are thawed before using in the procedure. IUI is the first line treatment for unexplained infertility along with ovulation-inducing medications.

3. Mild Male Factor Infertility:

IUI is also recommended if a man suffers from male infertility issues such as low sperm count and motility. Sperms are analyzed, washed, and treated to separate highly motile, normal sperm from those of lower quality. These sperms are then placed in uterus of a woman to assist fertility.

4. Semen Allergy:

In rare cases, women are allergic to proteins in her partner's semen. Contact with semen may cause redness, burning sensation and swelling. IUI treatment can be performed if the sensitivity is severe, as many of the semen proteins are removed before the sperms are inserted.

5. Cervical Factor Infertility:

Mucus produced by cervix is very important during ovulation as it provides an ideal environment for sperm to travel through vagina to fallopian tubes. IUI is also performed if cervical mucus is too thick as it may impede a sperm's journey to egg. During IUI treatment, the sperms are deposited into the uterus, increasing the number of sperms reaching the egg.

Drugs Used During IUI Treatment

You cannot initiate an IUI procedure or IUI treatment immediately as it takes planning and typically starts with the beginning of menstrual period along with a dose of hormonal drugs.

The doctor decides the type of drugs an IUI patient must take. For some women, if the timing is good and the eggs are healthy, there is no need for drugs to start IUI treatment whereas for others, they may require varying doses of Clomid, Gonadotropins or stronger medication to help them ovulate or to delay ovulation until their eggs are mature. Some women are advised to take injections before IUI procedure. Before taking any injectable drugs, a woman goes through a baseline ultrasound and blood work. Injectable medications are started on the day after all the tests are completed. The injections are of two types: Subcutaneous injections and Intramuscular injections.

1. Subcutaneous Injections: These shots are common hormonal fertility drugs which are given below the skin into the fatty tissue that lies between skin and muscle underneath. These injections do not hurt, although sometimes they can cause bruising. Subcutaneous injections can also be self-administered before infertility treatment.

2. Intramuscular Injections: These shots are given into the muscle directly. Patients may need to take these intramuscular shots before infertility treatment. Women can take the help of a nurse while taking these injections so that the process is over quickly.

Infertility Treatment, Ovulation

Clomid, Gonadotropins, and hcG Drugs

Fertility drugs improve the chances of conceiving with IUI as they stimulate ovaries to produce more eggs than in a normal cycle. Some fertility doctors prescribe Clomid, or clomiphene citrate, during a cycle. The timing of the IUI with a Clomid cycle can be perfected with urinary ovulation prediction kits or using ultrasound monitoring. If Clomid doesn’t help to produce eggs, the doctor may recommend gonadotropin injections, which are used to stimulate the hormones responsible for stimulation of follicles and egg production. Gonadotropin injections are also known as Follicle Stimulating Hormone or FSH.

After the tests are completed and injections are given to the patient, the monitoring phase begins where blood work and ultrasounds are performed three to four times by a fertility specialist. This is to monitor the growth and development of eggs. Once the lead egg, also called a follicle, reaches a certain size, women are given an injection of Human Chorionic Gonadotropin (hcG) which causes ovulation. IUI is performed one or two days after hcG injection is administered.

Monitoring Performed During IUI Treatment

IUI cycles require various monitoring sessions. The monitoring phase is divided into three different levels.

1. First Level of Monitoring:

Women need to track the surge of luteinizing hormone (LH) using Ovulation Predictor Kits (OPKs). During these days women are recommended not to do anything unusual that may disturb eggs. When a surge of LH levels is experienced, the patient is immediately asked to visit the concerned doctor. This level of monitoring is most common with un-medicated cycles.

2. Second Level of Monitoring:

During the second level of monitoring session, the patient is aware about the approaching big day. A doctor may monitor the hormone levels once or twice before ovulation and IUI treatment. A transvaginal ultrasound is also performed to confirm the size of the eggs. On this day, the patient is given special injections to ensure ovulation so that the IUI treatment can begin the next day.

3. Final Level of Monitoring:

The blood levels and vaginal ultrasound is almost performed daily to predict the right time for the deposition of sperms. At this time, the ovulation cycle is heavily medicated and the doctor begins to prepare for insemination as the procedure can begin anytime soon. When the time is perfect and eggs are mature, donor sperms or sperms of male partner are washed and cleaned and inseminated inside uterus via cervix. A thin tube is used to place the sperms.

Some women find the process uncomfortable and they may experience some cramping and spotting during the day after insemination. After the treatment, the patient is asked to lie down for a while and later allowed to go home. Women are advised to be watchful for any symptoms of pregnancy. Within 14 days the results of successful pregnancy may start to appear.

Success of IUI depends on several factors such as female’s age, reason for infertility, and whether fertility drugs were used. The success rate may reach as high as 20 percent if a couple performs an IUI procedure each month. As IUI is a less invasive and less expensive option, pregnancy rates from IUI are also lower than those from IVF.
